PAC contact info submission - 2018-2019

A member of each school's PAC executive should complete this form on an annual basis to ensure DPAC has current contact information for each PAC.

Each school has one vote exercised through its DPAC representative. Your DPAC Representative:

  • Should be elected from your PAC for the current school year.
  • Is responsible for attending DPAC meetings, exercising your PAC’s vote, and reporting back to your PAC.
  • Your PAC chair/co-chairs/vice-chair (as listed below) can assume the duties of your DPAC representative in their absence.

By providing my name below, I declare that I have read and understand the Use of Information and consent to the release of the contact information provided below, under the terms herein set out.



Use of Information:
Vancouver DPAC will use the information provided above to communicate information to you and other VSB parents
and guardians. Your contact information will be made available to the DPAC Executive, DPAC representatives, and PAC
Chairs and will be added to DPAC’s mailing list (MailChimp). DPAC will not share your contact information outside of the
school district without express consent. Vancouver DPAC will use the information provided for purposes pertaining to
DPAC or for the provision of information deemed by DPAC to be of interest to parents and guardians of students in the
District.

The information you provide will be used for a period of 16 months after the date of consent AND no later than
December 31 of the following school year with the exception of MailChimp which you can unsubscribe from at any time.
Consent may be withdrawn at any time upon written notice to DPAC. Please contact the DPAC chair at
